چکیده

CORTICOSTEROIDS HAVE PROVED to be very effective for the immunosuppressive treatment of organ transplant patients, but unfortunately they are responsible for many undesirable side effects, such as cataracts, delayed growth in children, skin fragility, bone diseases, and other manifestations considered to be cardiovascular risk factors, such as glucose intolerance, arterial hypertension, and dyslipemias. The reduction in the incidence of acute rejection in patients treated with mycophenolate mofetil in combination with anticalcineurin agents (cyclosporine or tacrolimus) has led to the design of studies to reduce or suppress corticosteroid treatment. We analyzed the 2-year course of cadaveric kidney transplant patients treated with a triple therapy of tacrolimus, mycophenolate mofetil, and corticosteroids in whom steroid therapy was withdrawn after the 3rd posttransplant month.

Prospective Controlled Protocol for Three Months Steroid Withdrawal with Tacrolimus, Basiliximab, and Mycophenolate Mofetil in Renal Transplant Recipients

During the past few years, new immunosuppressants, such as tacrolimus, mycophenolate mofetil (MMF) and basiliximab, have been shown to successfully decrease the incidence of acute rejection, possibly acting as potent substrates for safe steroid withdrawal. Therefore, clinical outcome of 3 months steroid withdrawal, while using the above immunosuppressants, was analyzed. Clinical trial registry ...

A large, prospective, randomized, open-label, multicentre study of corticosteroid withdrawal in SPK transplantation: a 3-year report.

BACKGROUND Simultaneous pancreas-kidney (SPK) transplantation is the treatment of choice for selected diabetic patients. Corticosteroids are an important element of immunosuppressive protocols, but their long-term use has detrimental effects on patients' health, necessitating eventual discontinuation. METHODS This prospective study evaluated the safety and feasibility of corticosteroid withdr...

Steroid withdrawal in renal transplant patients on triple therapy with a calcineurin inhibitor and mycophenolate mofetil: a meta-analysis of randomized, controlled trials.

BACKGROUND Two previous meta-analyses of randomized, controlled trials of steroid withdrawal after renal transplantation have shown significant increases in acute rejection (both analyses) and graft failure rates (the last analysis). A new examination of this topic including only randomized, controlled trials based on currently used, new, potent therapy with calcineurin inhibitors and mycopheno...

BENEFICIAL EFFECT OF LOW DOSE CYCLOSPORINE WITH MMF (MYCOPHENOLATE MOFETIL) IN RENAL ALLOGRAFT RECIPIENTS

Background: Calcineurin inhibitors (CNI) have significantly reduced the incidence of acute rejection. Nephrotoxicity however may contribute to long-term allograft dysfunction. Mycophenolate mofetil (MMF) may allow cyclosporine (CsA) dose reduction without increasing the risk of rejection. Methods: In seventy-eight living unrelated kidney transplant patients with renal dysfunction, we studi...

Improved growth and cardiovascular risk after late steroid withdrawal: 2-year results of a prospective, randomised trial in paediatric renal transplantation.

BACKGROUND Long-term corticosteroid treatment impairs growth and increases cardiovascular risk factors. Hence, steroid withdrawal constitutes a major topic in paediatric renal transplantation and maintenance immunosuppression. METHODS The lack of data from randomised controlled trials caused us to conduct the first prospective, randomised, multicentre study on late steroid withdrawal among pa...
